At Meridian Urgent Care, the receptionist may ask you a few questions about the nature of your problem or illness. Please do not feel they are prying. The information they obtain is vital to taking care of you in an appropriate manner. If you feel you do not want to be overheard, please ask to step into one of our private areas.

We make every effort to see our patients promptly in the order of arrival. Sometimes, however, emergencies occur which may delay your care. Also, at times we may see certain patients ahead of others when our staff feels their condition requires urgent attention. We appreciate your patience and understanding in these situations.

It is our policy to keep your situation confidential and your records securely stored. In the event you wish for us to communicate your history with us to other care providers simply call the facility you visited last and every effort will be made to accommodate your request. Insurance carriers require paperwork. We keep this private as well but do ask that you have a copy of your current insurance eligibility cards with you. You must present health insurance eligibility verification, photo I.D. and complete a patient intake form each time you are seen. Patients’ information changes frequently and it is necessary for us to have you complete all of the requested paperwork in its entirety. Please do not feel it is redundant – it’s for the accuracy of your file that we require this.

In addition to our privacy policy, Meridian Urgent Care provides same day, immediate care for non-life threatening conditions. Our staff of well-trained physicians, registered nurse practitioners, physician assistants, licensed technicians and medical assistants all provides friendly, personalized and professional care.

PATIENT RIGHTS

Meridian Urgent Care & Occupational Health Center Inc., physicians are committed to partnering with you in maintaining your good health. As a patient, you have the right to:

  • Exercise these rights without regard to gender, sexual orientation or cultural, economic, educational or religious background.
  • Receive information about our Medical Group, its services, and health care providers.
  • Be treated with professionalism, respect, courtesy and dignity.
  • Have all matters considered in privacy and confidentiality.
  • Participate in decisions about your healthcare and treatment, as well as receive adequate information about your diagnosis and proposed treatment plan from your healthcare provider.
  • A candid discussion of appropriate or medically necessary treatment options for your condition, regardless of cost or benefit coverage.
  • Refuse any procedure or treatment if you so desire and be told what effect this may have on your health.
  • Be informed of test results in a timely manner.
  • Obtain a second opinion by another provider.
  • Have the physician whom you visit focus his or her best efforts on your behalf in order to make a diagnosis and develop a proposed treatment plan based on the information available at the time of the visit.
  • Receive complete information about our fees upon request.
  • Right to voice complaints or appeals about the care provided.
  • Have the right to submit to the provider an amendment to the medical record if upon review the patient believes any item or statement is incorrect or incomplete.
  • Right to be represented by parents, guardians, designated family members or other conservators for those who are unable to fully participate in their treatment decisions.
